Description<\/b>: In recent years there has been significant national recognition and growth related to improving patient outcomes and increasing patient safety in healthcare. The Institute of Medicine (IOM, 1999) reported an estimated 98,000 people die each year in hospitals related to preventable medical errors in the publication To Err is Human<\/em><\/a><\/span> .<\/span><\/i>\u00a0Since this publication, many national organizations, programs, and campaigns have developed over the last 10-15 years, striving to improve healthcare safety, including the 5 Million Lives Campaign<\/a>, Joint Commission National Patient Safety Goals Program<\/a>, National Quality Forum<\/a><\/span>\u00a0for patient safety in healthcare, and Quality and Safety Education for Nurses (QSEN)<\/a>. This badge recognizes individuals have recognized the importance of continued education about maintaining safe and quality patient care. <\/span><\/p>\n

Criteria<\/b>: \u00a0Individuals who earned this badge have completed three activities. \u00a01.) View <\/a><\/span>Chasing Zero: Winning the War on Healthcare Harm<\/span><\/i> video (53 min) available from the QSEN website.<\/span>\u00a02.) Read and review the IOM Future of Nursing Report (2011) Chapter 3 Transforming Practice<\/a>\u00a0<\/span>\u00a03.) Complete the Patient Safety modules through the Institute for Healthcare Improvement<\/a>: PS 101, PS 102, PS 103, and PS 104. You must create a login (free) to access and complete the modules.\u00a0<\/span><\/p>\n

After completion of each module the learner will be able to download a certificate of completion. Please submit the certificate of completion for Patient Safety modules 101-104<\/span>\u00a0 in electronic or paper copy to Dr. Jaime Hannans. The student must be in good standing in the Nursing Program at the time they have met the criteria to be awarded this badge.<\/span><\/p>\n

Evidence<\/b>: <\/span>To earn the badge, students need to provide the acknowledgement (Form<\/a><\/span>) of completing tasks* and have submitted the post activity reflection discussion response.<\/span><\/p>\n
